General Information:

  • Is a deficiency of TH resulting in the slowed body metabolism, decreased heat productiion, and decreased oxygen consumption by the tissues.
  • Hypothyroid state resulting from hyposecretion of thyroid hormones T3 and T4
  • Occurs often in women between ages 30 and 60

Assessment Findings:

  • Cardio: Bradycardia, cardiac complications
  • Musculo-skeletal: Lethargy, syncope, fatigue, weakness, muscle aches, paresthesis, slow clumsy movements
  • Genito-Renal: Weight gain, menstrual irregularities
  • Neurologic: Forgetfulness and loss of memory
  • Integumentary: Generalized puffiness and edema around the eye and face (myxedema), Intolerance to cold, dry and scaly skin, sparse hair, brittle nails
  • Digestive: anorexia, constipation
  • Diagnostic Exams: low Serum T3 and T4, elevated Serum cholesterol, decreased RAIU

Etiology and Risks Factors:

  • Congenital defects of thyroid (cretinism)
  • Defective hormone synthesis
  • Iodine deficiency
  • Anti-thyroid drugs
  • Chronic inflammatory diseases

Nursing Interventions:

  • Monitor vital signs including heart rate and rhythm, monitor I and O, weigh daily
  • Observe for edema and signs of cardiovascular complications
  • Administer thyroid replacement: Levothyroxine Na (Synthroid) and observe signs of thyrotoxicosis (tachycardia, diarrhea, palpitations, nausea and vomiting, sweating, tremors, agitation and dyspnea)
  • Provide comfortable and warm environment
  • Instruct client in low-calorie, low cholesterol and low fat diet.
  • Institute measures to prevent skin breakdown
  • Provide increased fluids and foods high in fiber to prevent constipation
  • Instruct client to take thyroid hormone replacement in moring to prevent insomia

Medical Management:

  • Drug therapy: levothyroxine (Syntheroid), thyroglobulin (Proloid), dessicated thryroid liothyronine (Cytomel)
  • Correct Thyroid deficiency
  • Reverse manifestation
  • Prevent cardiac complications
